Claiming Credit and Participation

In order to receive continuing education credit, you must attend and/or view each session for which you would like to receive credit. Please find below the CE Claim Form as well as the conference evaluation. Both forms must be completed in order to receive credit.

REMINDER: Each Continuing Education Session will have a code associated with it. A four digit code will be provided during each session. Please note that both forms require one submission and are not able to be saved, so plan to complete each form at one time.
